Making An Appointment When calling to request services, you will first talk with the Intake Support Specialist, to whom you will give information such as your name, address, telephone number, social security number, etc. You will also provide information concerning finances, employment and insurance, so that the cost of treatment can be determined. The Intake support Specialist will then arrange your first appointment. for most clients, this will include a brief discussion of the presenting problem, the completion of a personal/medical history form and the financial paperwork. Once these steps have been completed, you will be scheduled for an hour assessment appointment. After completing the assessment, appropriate treatment will be arranged with professionals experienced in the areas necessary for your treatment. Marion
